Patient Connections Manager – Cancer Care Navigation

About the role

The Patient Connections Manager leads the on‑ground patient navigation team in Freeport, handling both personal caseloads and team leadership. You will translate complex oncology science for families, ensure rapid response standards, and build FCCT’s local presence.

Key responsibilities

• Complete FCCT’s foundational curriculum and master the case‑management workflow (Airtable, supplier reference, documentation standards).
• Maintain a personal caseload of families across cancer journeys, providing scientific translation and 24‑hour follow‑up.
• Lead and coach the Freeport Associates team, overseeing daily case reviews, learning sessions, and performance.
• Enforce FCCT standards: 24‑hour family response, documentation, and monthly playbook contributions.
• Escalate complex clinical questions and handle operational issues directly.
• Partner with the Director of Patient Navigation on hiring, performance management, and functional evolution.

Required profile

• Proven formal management experience (hiring, coaching, performance reviews) in a clinical or research setting.
• Advanced scientific background: Master’s degree or higher in a biological/chemical science with hands‑on research experience (thesis, publication, or practicum).
• Ability to communicate complex scientific information clearly in writing and verbally.
• Emotional resilience to handle difficult conversations and sustain team morale.
• Commitment to reside in Freeport long‑term; Bahamian nationals preferred, with sponsorship possible for exceptional candidates.
